Hello, I’m Tacara Soones, MD.

I’m a Harvard-educated, board-certified Internist and Geriatrician in Houston, TX.
Helping my parents manage the complexities of aging for 3 of my grandparents helped me understand firsthand the ways that our healthcare system falls short for seniors and their families. It's hard to see a doctor who knows you. Visits are rushed. Families are overwhelmed. I opened Dahlia to provide the missing primary care ingredient- a trusted, physician navigator.
Dahlia Physician Housecalls is a membership-based primary care practice designed for seniors and their family members who want a concierge geriatrician in Houston, TX who provides home visits. As a physician who specializes in the care of seniors, my goal is to elevate the well-being of older adults by not only managing chronic health issues but also developing personalized, proactive care plans that help seniors live more comfortably, independently and with dignity.
Unlike a traditional primary care practice of 2000 patients, Dahlia is limited to a small number of older adults. You and your family deserve a doctor who knows you and is dedicated to a better health care experience with more time and less stress.
