General Information
-
Concierge medicine, or membership-based primary care, is a healthcare model where patients pay for personalized, proactive care without the limitations of insurance. At Dahlia Physician Housecalls, our membership includes annual and follow-up, routine home visits supported by ongoing communication and education. Our members benefit from high-quality, convenient healthcare.
-
A geriatrician is a doctor who specializes in caring for older adults, addressing the unique health needs that come with aging. You might benefit from a geriatrician if you or a loved one:
Has multiple chronic conditions.
Take several medications and need careful management.
Are homebound or have difficulty traveling to a doctor’s office.
Want comprehensive care focused on what matters most to you.
At Dahlia Physician Housecalls, our geriatric care is tailored to seniors, providing in-depth home visits that help manage complex health concerns and support both patients and caregivers.
-
We come to you, whether at your residence or assisted living facility. This is ideal for older adults who prefer to avoid traditional office settings. We'll coordinate appointments to fit your schedule and deliver the same level of comprehensive care as you'd expect in a clinic.
Our housecalls service is available within a 25 minute drive of the Texas Medical Center in Houston, TX. This includes most parts of Harris County. If you live outside of that area, please reach out. We may be able to accommodate you.
-
No, Dahlia Physician Housecalls is not a home health agency. We provide concierge medical care for seniors through house calls. While home health agencies offer nursing and therapy services prescribed by a doctor, Dahlia provides:
Comprehensive medical care directly from a doctor.
Routine home visits to manage chronic conditions, medications, and overall health.
Coordination of care with specialists, home health agencies, or other providers as needed.
Our goal is to ensure you receive exceptional, coordinated care where it matters most: at home.
-
Yes! The Medicare Independence at Home (IAH) Trial tested how in-home primary care could help patients with chronic conditions. The results showed that patients were more satisfied and hospital admissions dropped by 10%.
This trial showed that in-home primary care can improve health and save money. Dahlia Physician Housecalls is not only a boutique, personalized, proactive primary care practice but also evidence-based.
